How to update property-owner information To update property-owner’s address you must complete the following documentation: HCV Landlord Change of Information Form To update agent information you must complete the following documentation: Official signed letter from landlord indicating requested changes W-9 form (only if agent is listed as the payee) Agent Affidavit Assignment of New HAP Contract (only if agent is listed as the payee) Agent/Owner Contract To update banking information: Direct Deposit form Voided Check Owner of Property Transition If there is a change in the owner of the property or the Tax Identification Number, the following requirements must be completed. Official signed letter indicating change Assignment of New HAP Contract W-9 form from owner W-9 form from agent (If applicable) Agent Affidavit (If applicable) Agent/Owner Contract (If applicable) Assignment of Tenant information Direct Deposit Authorization Agreement Current Rental License(s) of all associate properties HUD 1 Settlement sheet or Deed of Records of all associate properties Submission of Photo Identification or owner and/or agent If applicable, the new landlord needs to complete the IREM course If applicable, the new landlord needs to attend the Landlord Briefing Session.
