  • W-9 form (only if agent is listed as the payee)
  • Agent Authorization Form
  • Assignment of New HAP contract (only if agent is listed as the payee)

Certificates and Brochures

The Philadelphia Property Maintenance Code (Section PM-102.6.4) requires an owner offering residential property for rent, to provide residents with a Certificate of Rental Suitability and a copy of the “City of Philadelphia Partners for Good Housing” brochure issued by the Department of Licenses and Inspections.

Programs

Landlord Cooporation Program (LCP) â€“ PGW offers this program to Philadelphia property owners. Property owners who provide complete cooperation and compliance with LCP are not subject to the placement of liens on registered properties.
